Male - $2.60 Monthly

Female - $5.75 Monthly

Male -   $2.30 Monthly

Female -  $5.00 Monthly

 

* Above rates apply to groups under 50 employees

* Minimum quarterly premium of $15.00 ($60.00 annually)
